L4-L5 Disc Bulge – The lower back is where bulging and herniation of discs happen most often, especially in the L4-L5 area. This part of your spine is right at your beltline and helps you bend and twist about 95% of the time. Since it carries most of the upper body’s weight, it experiences a lot of pressure. What is L4 L5 Disc Bulge?

Our spine is made up of small bones called vertebrae, and between them are soft discs that work like cushions to absorb shocks and help the spine move easily. A disc bulge, which is also called a slipped disc or disc herniation (though they are slightly different), happens when the soft jelly-like centre of a disc (nucleus pulposus) pushes through its tough outer layer (annulus fibrosus). This happens because of a weak area at the back of the annulus fibrosis. If the bulging disc is between the fourth (L4) and fifth (L5) bones in the lower back, it is called an L4-L5 disc bulge.

Here are some common symptoms of an L4-L5 disc bulge:

Lower Back Pain

One of the most common signs is pain in the lower back. You can feel normal pain or sharp pain and it may also get worse when you sit, bend, or lift heavy things.

Leg Pain (Sciatica)

If the bulging disc presses on the sciatic nerve-involved nerve root, it can cause pain that spreads down the leg. This pain may start from the lower back and go through the buttocks, thigh, and even down to the foot.

Numbness or Tingling

Many people with an L4-L5 disc bulge feel numbness or tingling in their lower back, legs, or feet. It may feel like pins and needles or a burning sensation.

Weakness in Legs or Feet

Some people can find it hard to lift their feet while walking. This is called foot drop and happens when the nerves that control the foot are affected. Foot drop or muscle weakness is a serious condition and needs immediate attention.

Pain That Gets Worse with Movement

Activities like walking, standing for a long time, coughing, and sneezing can worsen the pain.

Causes of L4-L5 Disc Bulge

Here are some common causes of an L4-L5 disc bulge:

Ageing – As people get older, the discs in the spine lose water and become less flexible. This makes them weaker and more likely to bulge or break.

Lack of Exercise – Not staying active makes the back muscles weak. Weak muscles cannot support the spine properly, making disc problems more likely.

Heavy Lifting – Another cause of L4/L5 disc bulge is lifting heavy objects the wrong way can put extra pressure on the spine. If you bend or twist while lifting, the disc may get damaged.

Sitting for Too Long – Sitting for long hours, especially with poor posture, can put pressure on the lower back. This can slowly weaken the disc at its posterior part and lead to a bulge.

Repetitive Movements – Doing the same movements again and again, like bending, twisting, or lifting, can wear out the disc over time. 

Injury or Accident – A sudden fall, car accident, or sports injury can damage the disc and cause it to bulge.

Obesity – Being overweight puts extra stress on the spine, which can lead to disc problems.

L4-L5 Disc Bulge Treatment

As we discussed before, an L4-L5 disc bulge happens when the soft disc between the fourth (L4) and fifth (L5) bones in the lower back pushes out. This can cause pain, numbness, or weakness in the lower back and legs. There are different ways to treat this problem, depending on how serious it is.

1. Rest and Lifestyle Changes

If the pain is not too bad, resting for a few days can help. Strict bed rest however is not recommended and it is advised to walk around a little. Also, avoid lifting weights, bending too much, or sitting for a long time. Using good posture while sitting and standing can also reduce pain.

2. Pain Relief Medicines

Doctors may suggest pain relievers like ibuprofen or acetaminophen to reduce pain and swelling. These medicines should only be taken as directed. Additionally, another class of medicines, called pain modulators, provide benefits in pain related to nerve compressions.

3. Physical Therapy

A physiotherapist can teach exercises to strengthen the back and core muscles. Stretching and strengthening exercises help take pressure off the bulging disc and reduce pain.

4. Hot and Cold Therapy

Applying a warm pack or ice pack to the lower back can help reduce pain and swelling. Ice is good for fresh pain, while heat helps relax muscles.

5. Spinal Injections

If the pain does not go away with other treatments, a doctor may give epidural injections around the affected nerve root to reduce inflammation and pain related to bulged discs.

6. Surgery (For Severe Cases)

If the disc bulge is very bad and does not improve with other treatments, surgery may be needed. A doctor may remove the part of the disc that is pressing on the nerve. This can be performed either as an endoscopic procedure or an open surgery.
